The Health Navigation Network (HNN) provides support to patients and families with hospital discharge planning, by offering a comprehensive framework for seamless transitions from hospital to home or alternative care settings.
We are able to facilitate the completion of assessments, ensuring thorough evaluations of patients' medical, functional, and social needs; to effectively match patients with a choice of appropriate care services that align with their individual requirements, choices and expectations.
We also endeavour to work alongside the care providers and operate within a timeframe that synchronises with hospital discharge planning (if possible), minimising delays and ensuring timely access to necessary care services.

Falls Prevention & Private Physiotherapy Support
The Health Navigation Network works with a team of experienced Community Falls Specialits and Physiotherapists.
This is a remote service, completed via virtual or telephone consultations to understand your needs, difficulties or concerns and provide bespoke plans to:

Support recovery following NHS rehabilitation completion or hospital stay
Advise on staying safe in and around the home
